A St John Ambulance spokesperson said it appeared there were "no life-threatening injuries", with two male toddlers and a woman in her 30s being treated at the scene.

A female teenager and another woman in her 30s has been taken to Midland Hospital, while a sixth person, a woman in her 50s, has been taken to Royal Perth Hospital.

Eyewitnesses have described a large contingent of first responders in Guildford's town centre.

Police said they had received reports of the collision on Meadow Street just before 4pm, and emergency services had responded immediately.

Eyewitness Eric told the ABC a truck had been hit by a train, describing the scene as "pretty chaotic".

He said the truck appeared to have been torn in half.

"The truck's in two ... from what I could see the cab of the truck's further across the railway crossing and the tail part of the truck was laying just prior," he told 102.5 ABC Perth.

Pictures from the scene appear to show a truck with a metal tray stuck on the tracks, with boom gates nearby bent out of shape.

Another eyewitness said the train "flew into it [the level crossing] at speed, tore off an axle from the truck which has landed on a four wheel drive."

The front windscreen of the four wheel drive appeared to have been smashed and its airbags deployed.

Another eyewitness Robin Woodward, who lives nearby, said the level crossing had been "partially destroyed" by the truck, which had its trailer ripped off when it was struck by the train.

He said one of the trailers on the "semi-trailer kind of a truck" had been "completely cleaned up" and a bobcat on the trailer ended up on the train tracks.

He said paramedics were tending to two women, who appeared to be in shock, one of whom was in an ambulance.

Mr Woodward said the train had been significantly damaged, as had an SUV at the level crossing.

"The whole front of the train, the windows are completely smashed and one side of it … damaged," he told 102.5 ABC Perth.

"The back axle has landed on the bonnet of this SUV and it's destroyed that."

Trains on the Midland line have been cancelled between Bayswater and Midland as a result, with replacement buses running.

'Apocalyptic' scene

Passengers were helped off the three-carriage train by Transperth officials, and described the moment of impact.

"We were just having a conversation, then the train started shaking and we fell on the floor and .. there was dust everywhere," one young woman told reporters.

She said passengers' belongings were scattered and one man had injured his back.

Transperth said that was also causing delays on the Airport, Ellenbrook and Fremantle lines.

Traffic is also heavy in the area.

The same Meadow Street level crossing was the site of a fatal incident in 2015, when a track worker was fatally struck by a passenger train.
