葉宗勳說明,不管是美國心臟協會(AHA)還是歐洲心臟學會(ESC),在所有國際高血壓臨床指引的首要前提是,飲食控制與體重管理。每減少1公斤體重,血壓就能隨之下降,這是最天然也最有效的良藥。
他接著說,許多患者面對「少油、少鹽、控糖、減重」等生活調整建議,往往認為每天吃藥比較簡單,卻忽略生活習慣也是控制血壓的重要一環。張先生將醫師的建議落實在生活中,經半年努力減重10多公斤,血壓也持續維持在理想範圍。

●切勿自行停藥或調藥
體重下降後血壓通常會隨之改善,但絕不可自行停用或減少降血壓藥物。建議每日定時記錄血壓(可參考「722原則」),若收縮壓持續低於100 mmHg或出現頭暈等症狀,應攜帶紀錄回診,由醫師評估調整藥量。
●運動避免過度憋氣
建議從快走、單車等中低強度有氧運動開始。血壓尚未控制穩定前,應避免猛烈重訓或長時間憋氣用力(華爾沙娃運動),防止血壓驟升。
●飲食均衡並注意高鈉與慢性病限制
採低鈉、高纖與均衡飲食(如得舒飲食原則)。若合併有慢性腎臟病或糖尿病,進行高蛋白或高鉀飲食前應先諮詢醫師或營養師。
